Blank W9 form: what it's for

Form W-9 (Request for Taxpayer Identification Number and Certification) gives a business your correct name and taxpayer ID so it can report payments to you, for example on Form 1099-NEC. Download the official blank form from IRS.gov; you give the completed form to the requester, not to the IRS.

Filling it in, line by line

  1. Line 1: your name as shown on your tax return (for a disregarded single-member LLC, the owner's name).
  2. Line 2: business name or DBA, if different.
  3. Line 3a: federal tax classification (individual/sole proprietor, C corp, S corp, partnership, trust/estate or LLC with its tax code).
  4. Lines 5–6: address. Part I: your SSN or EIN. Part II: sign and date.

Backup withholding

If you don't provide a correct TIN, or the IRS notifies the payer, the payer must withhold 24% of reportable payments as backup withholding. Only send a W-9 to someone you're actually doing business with; never email your SSN to an unexpected request.
